Required and Optional Return to School Forms

Below you will find the forms you will need to start the new school year. This year completion of forms to return to school will be completed online.

Please complete all of the required forms by October 16, 2020.

Emergency Care Card Form and Health Information

REQUIRED FORMS FOR ALL STUDENTS

Emergency Care Form

  • All students must have accurate emergency care information. Parents update this information through WeCare@School via their SIS ParentVue account. In order to log into Parent View it is necessary to have an active SIS Parent Account. If you need a SIS Parent Account, please contact Eileen McGarry at [email protected].  If you are new to FCPS you will receive an activation letter in the mail.
    • Log into your SIS ParentVUE account.
    • Go to FCPS Links located in the left-hand menu.
    • Go to FCPS 24/7 and Log in using the same username and password as your SIS ParentVUE account.
    • Go to WeCare @ School on the top right of your screen.

  • Teachers will use a variety of digital resources to support student learning and promote communication and collaboration with students and families. Some of the digital resources your child may use this year will require your consent before they can be used. You can find our school’s list of the resources that require parental consent here https://newingtonforestes.fcps.edu/adopteddigitalresources

    Included on this page is also a link to a form where the enrolling parent or guardian will need to indicate whether or not their student is allowed to use the list of digital resources this year. This page also provides more information about how FCPS carefully reviews digital resources for approval and why we must request your consent before using certain tools. We encourage you to complete the parental consent for digital resources form as soon as possible

Student Rights and Responsibilities (SR&R)

Parents and guardians, please review the Student Rights and Responsibilities and related resources at www.fcps.edu/srr. After reviewing the information, fill out the SR&R Parent Acknowledgement Google Form using the link we have provided.  The form must be filled out one time per student.

Parent acknowledgement is due October 16, 2020.

Please download and complete the form if it pertains to your student.  Return completed form to Ardenia Davis at @email or mail it to the school by October 16.

 

If you are not opting your child out of anything, no action is required.

If you do not wish for your child’s information to be published via the school directory, yearbook, website, or to the Fairfax County Public Library, please complete the Objection to Release of Directory Information to the Public form (page 11 & 12). If you do not wish for your child to participate in academic, career or social or emotional counseling, please complete the School Counseling Opt-Out Form (page 13)

We must receive completed opt out forms no later than October 16.

We ask that you review all options on the form carefully to avoid opting your child out of something you intended for them to participate in. Once these publications (school directory and yearbooks) are printed, changes cannot be made.

Please download and complete the Opt-Out Form only if they pertain to your student. If possible, please scan completed form(s) and email them to Ardenia Davis at @email or mail it to the school.

 

Fairfax County Public Schools (FCPS) provides a comprehensive, sequential Family Life Education (FLE) program for students in grades kindergarten through grade twelve. Instruction is seen as a partnership among parents and guardians, the school, and the community in supporting the learning essential to the development of strong families, positive relationships, and a healthy community.

Parents/guardians may choose to opt their child out of all or part of the Family Life Education program. Students who are opted out are provided with age-appropriate, nonpunitive alternative health instruction, and every effort is made to foster respect for family choices. If you wish to opt your child out of all or part of FLE for this school year, opt out forms are available online for Elementary (K-6)

Select the grade level FLE Opt-Out form for your child. Enter your child’s name, teacher name, check the line next to the lesson(s) you do not want your child to take part in, sign and date the form. If possible, please scan completed form(s) and email them to your child's classroom teacher. If you are unable to scan the completed form(s), please mail the form to the school.
